Our approach to working with people has always been to combine their immediate care needs with a goal of community re-entry. Through a commitment to “person-centered care,” we work with individuals and their families to develop a care plan that affords them the greatest degree of independence. To every extent possible, we identify and support the individual’s preferences for how to live their life. Personal growth, self responsibility and autonomy serve as the building blocks of the wellness plans we create for recovery following life-changing events.
The people we serve help to choose their care teams and those teams stay consistent. Nursing supervisors evaluate each new patient to coordinate an individualized care plan with the physician and family. They hold regular meetings with the physician and other providers, conduct training with field staff and family members, perform hands-on patient assessments, and find community resources; ours is a true care team approach.
